To have additional children ID'd, contact the doctor listed on the back of your existing Kid ID Card. If you've lost your card you can also order a replacement card through that same doctor's office.

If you have not received your ID card or your card has wrong information please click here, download the form provided, print and fill out the information and mail to Kid ID at the address provided.
You must return the old card before we will send you a new one.
